Is low CTR always a title problem?

No. Rank ceiling, zero-click SERPs, and branded queries that are already fine are common. Titles matter when the tag points at presentation, not when position or the SERP layout is the real ceiling.

When should I rewrite title or meta?

What is a zero-click SERP?
